Planning of the 3rd Annual Wild Hog Music Fest & Motorcycle Rally is
Festival dates are April 25, 26, and 27, 2003.
I would like to offer you the opportunity to apply for food concession space at the festival.
After reviewing all the applications, Wild Hog Music Fest & Motorcycle Rally Food Vendor committee will select the vendors to sell at the 3rd annual festival. Please remember that you will be charged for space including outside space for cookers, trailers and any other equipment used to run your operation. The Wild Hog Music Fest & Motorcycle Rally must approve all uses of space in advance.
Included with this application is a summary sheet. The Wild Hog Music Fest & Motorcycle Rally Regulations and Requirements and Health Department Guidelines. Please read each item carefully then complete and sign the application.

I UNDERSTAND THE FOLLOWING:
A: Full payment must be sent with signed contract due March 17, 2003.
B. Proof of Commercial General Liability Insurance will be required no later than March 17, 2003.
C. I have read the above requirements of the Wild Hog Music Fest & Motorcycle Rally Food Vendor Regulations and Requirements, the Phillips County Health Department Guidelines for Temporary Food Service operations and the indemnification and agree to their stipulations. This application is true and accurate to the best of my knowledge.
